Sriwijaya Air Flight 182 was a scheduled domestic passenger flying from Dki jakarta to Pontianak, Indonesia. Five minutes afterwards departing from Soekarno–Hatta International Airport on 9 January 2021, the Boeing 737-500 experienced an upset and crashed into the Java Sea off the Thousand Islands, killing all 62 people on lath. A search of the surface area recovered wreckage, human remains, and items of clothing. It is the third deadliest crash involving a Boeing 737-500, behind Asiana Airlines Flight 733 and Aeroflot Flight 821[5] The flight data recorder (FDR) was recovered on 12 January, and the data storage module of the cockpit voice recorder was recovered on 30 March.[6]

During the search, the National Transportation Safety Committee (NTSC) used the engines and bachelor information from Flightradar24, and hypothesised that the plane was intact upon impact.[ failed verification ] It was known that the autothrottle on this aircraft had malfunctioned a few days before, and one line of investigation was whether this might have contributed to the blow.[seven]

A preliminary study released on 10 Feb 2021 discovered problems with the aeroplane's autothrottle, where the left thrust lever engine started to reduce as it climbed, while the right thrust lever engine remained the same.[8]

Prior to Flight 182, the aircraft arrived at Soekarno–Hatta International Airport in Tangerang, Banten at 12:eleven PM from Pangkal Pinang Depati Amir Airport.[9] The shipping was scheduled to take off at 13:25 WIB (06:25 UTC), and was scheduled to arrive at Supadio International Drome in Pontianak, W Borneo, at 15:00 WIB (08:00 UTC). After pushing back from the airport'south Concluding 2D Gate B1,[x] the shipping took off from Rail 25R at fourteen:36 local time (07:36 UTC).[11] The flight took off amid heavy monsoon rain, post-obit a bad weather delay.[12] Due to the significant filibuster, it was expected to land in Pontianak at 15:l WIB (08:50 UTC).[10]

Flight 182 was climbing through ten,900 ft (3,300 k) at fourteen:40:05 when it experienced an upset and rolled to the left[8] [13] An air traffic controller (ATC) noticed this and asked the pilots what was happening on lath, but received no response.[14] Co-ordinate to AirNav Radarbox flight data, the aircraft reported that the climb phase of the flight was interrupted by a rapid driblet in altitude from x,900 ft (iii,300 m) to 7,650 ft (2,330 m) at 07:twoscore UTC.[15] Flightradar24 reported that four minutes after takeoff, the aircraft dropped by ten,000 ft (3,000 m) in less than a infinitesimal.[16] The flying tracker besides noted that the aircraft's last recorded altitude was 250 feet (76 m) at 07:40:27 UTC.[17] Co-ordinate to flying information provided, the airplane experienced a drop of 1,755 ft (535 chiliad) in merely six seconds between 07:forty:08 and 07:40:14 UTC. It was followed by a drib of 825 ft (251 m) in two seconds, 2,725 ft (831 g) in iv seconds, and five,150 ft (1,570 m) in its last seven seconds.[18] During the autumn, the aircraft rapidly changed speed, decreasing and increasing in seconds.[19] Its final contact with ATC was at 14:twoscore WIB (07:40 UTC) in a location betwixt Laki Island and Lancang Island.[20]

The aircraft crashed into the Java Sea near Laki Isle and 19 km (12 mi) from Soekarno–Hatta International Drome.[9] [21] There were no survivors. Co-ordinate to the ATC, there was no distress call during the flight. Indonesian transport officials also stated that the shipping failed to follow ATC instructions.[22]

Aircraft [edit]

The shipping involved was a Boeing 737-524, registered as PK-CLC (c/n 27323/Line no.2616).[23] Information technology was equipped with ii CFMI CFM56-3B1 engines.[24] At the fourth dimension of the accident, the aircraft had accumulated 62,983 hours and xl,383 cycles.[25]

The aircraft was manufactured in 1994, and had its maiden flight on xiii May 1994. Information technology was first delivered to Continental Airlines on 31 May 1994 under the registration N27610. The aircraft was transferred to United Airlines in 2010 later on Continental and United merged. On 15 May 2012, the shipping was sold to Sriwijaya Air. It was the kickoff of a full of fifteen 737-500s received by Sriwijaya Air in 2012 to replace their 737-200s.[26] [ix] Sriwijaya Air named the aircraft Citra.[24]

Betwixt 23 March and 23 October 2020,[27] the shipping was stored at Surabaya Juanda International Airport for repair.[28] [9] The Ministry of Transportation stated that it inspected the shipping on 14 December 2020 and issued a new certificate of airworthiness on 17 December 2020. It resumed service on xix Dec 2020.[28] [9]

Passengers and crew [edit]

At that place were 62 people on board, of which l were revenue passengers (43 adults and 7 children).[29] Of the 12 crew members, half-dozen were operating crew on the flight, while the other six were deadheading as passengers. The majority of the passengers were residents from W Borneo.[xxx]

Among the passengers were Mulyadi Tamsir, a political leader from the People's Conscience Political party (Hanura) and chairman of the Indonesian Muslim Student Association (PB HMI),[31] [32] [33] makeup artist Syifa Mila,[34] and YouTuber Faisal Rahman.[35]

The coiffure on the flight consisted of Captain Afwan, 54,[a] who had over 17,904 hours of flight experience, nine,023 hours of them on the Boeing 737,[36] Offset Officer Diego Mamahit, who had over 5,107 hours of flight feel, four,957 hours of them on the Boeing 737,[36] and four flight attendants.[37] Afwan was a former pilot in the Indonesian Air Force.[38]

The six deadheading coiffure and several revenue passengers had transferred to Flight 182 from an earlier NAM Air flying that did non operate.[39] [40]

The Associated Press, BBC News, HuffPost, The New York Times, The Wall Street Journal, and other media accept reported that all people on board had been killed.[41] [42] [43] [44] At least 58 victims, including the helm, have been identified by the law.[45]

Search [edit]

Recovered droppings displayed in forepart of multiple rescue personnel every bit BASARNAS gave daily updates on the search and rescue performance

The first report of the crash was made at 14:30 WIB (07:thirty UTC), in which a fisherman said that an aircraft had crashed and exploded in the sea.[46]

The head of the Indonesian National Search and Rescue Bureau (Indonesian: BASARNAS), reported that the crash site was located xi nmi (20 km) from Soekarno-Hatta International Airport.[47]

Personnel from a vessel provided by the Ministry of Transportation reported that body parts, fragments of clothing, electronics, personal property and wreckage had been recovered from the body of water in waters nigh the Thousand Islands, with aviation fuel also reported around the location.[48] [49] The water almost the crash site has a depth of around 15–16 grand (49–52 ft).[50]

BASARNAS immediately deployed personnel to the crash site[51] while the Indonesian National Police and the Ministry of Transportation ready crisis centres in Port of Tanjung Priok[52] and Soekarno–Hatta International Drome.[53] The Indonesian Navy deployed a number of vessels for the search and rescue operations, in addition to helicopters and KOPASKA (frogman) personnel.[54]

The Indonesian government requested help from the Due south Korean authorities with the search. Through the Korea - Indonesia Marine Technology Cooperation Research Center (MTCRC), the South Korean Ministry of Oceans and Fisheries deployed a research vessel equipped with a detector to Indonesia. At least 15 personnel from MTCRC were also sent.[55] [56] The Singaporean authorities too offered to provide assist in the search which the Indonesian regime after accustomed.[57]

On 10 Jan, the Indonesian Navy announced that the exact coordinates of the crash site had been pinpointed.[58]

Recovery [edit]

KOPASKA showing efforts to search the remains

Rescuers managed to recover a life vest, pieces from the aircraft'south fuselage, and a destroyed cycle rim of the Boeing 737. Almost of the wreckage was establish at a depth of 17–23 metres (56–75 ft).[59] On the dark of ix January, an emergency slide of the aircraft was recovered from the waters near Lancang Island, Thousand Islands.[60] The scattered debris and the small pieces of the wreckage indicated a high-speed impact. Much more wreckage was constitute over the adjacent few days.[61]

On ten January, the National Transportation Safety Committee (NTSC) reported that they had located the position of the cockpit voice recorder and the flight data recorder,[62] [63] but later the flight data recorder was retrieved at 16:00 WIB on 12 January,[64] [65] it was reported that beacons on both flight recorders were dislodged in the impact and the cockpit voice recorder would need to be found without the assist from underwater guidance signals.[66] [67] The casing of the cockpit vocalism recorder was recovered on 15 Jan merely the data storage module within was missing.[68] BPPT stated that the information storage module of the CVR was thought to exist buried under the aircraft's wreckage.[69] In the afternoon of 17 January, the CVR components and its casing, but without the data storage module, were handed over to the NTSC for further examination.[70] The next solar day on 18 January, the search for the CVR, wreckage, and victims was extended for three more days.[71]

On 21 Jan BASARNAS announced the search for victims and aircraft debris had been halted.[4] A total of 122 pieces of debris had been recovered from the crash site, including one of the aircraft engines' turbines. As of 10 February, the search for the data storage module of the CVR was ongoing.[72]

On 30 March, virtually iii months after the crash, BASARNAS recovered the missing CVR component using a hopper dredger, at a depth of fourteen meters, with coordinates of 5°57′51″S 106°34′31″E / 5.96417°Due south 106.57528°E / -five.96417; 106.57528 ; the search operation was officially ended.[73] [74] [75]

Investigation [edit]

The NTSC was immediately notified of the accident, with assist from BASARNAS. NTSC stated that starting on 10 January 2021, simply before 06:00 local time, search and rescue personnel would first searching for the aircraft's flight recorders.[76] It added that the investigation will be assisted by the The states' National Transportation Safety Board (NTSB), with the Singaporean Ship Safety Investigation Bureau also offering assistance with the investigation.[77] [78] [25] On 10 January, NTSC obtained raw data of the aircraft'due south flight path from radar and interviews with the air traffic controller.[79] Investigators also retrieved the transcript of the communication between the pilots and the ATC.[80]

A spokeswoman from the Indonesian Ministry of Transportation reported that an abnormality was noted during the flying. The aircraft departed from Djakarta's Soekarno-Hatta International Airport on a standard instrument departure. The aircraft had been cleared to climb to 29,000 ft (viii,800 m). During its climb, Flight 182 went off class to the northwest. ATC asked the coiffure virtually the divergence and got no response. A few seconds afterward, the aircraft dropped off radar.[81] [82]

An investigator with NTSC stated that based on the distribution of the debris, the aircraft maybe ruptured when it striking the water.[83] Combining Flightradar24 data and the shape of the engine turbine'due south fan bract and turbine disc, NTSC speculated that the aircraft's engines were still operating upon bear on; based on the evidence, the aircraft was still responsive at 250 feet.[84]

There was a public concern that the aircraft was not airworthy.[85] The Federal Aviation Administration initially had issued an Airworthiness Directive to Boeing 737-500 operators, concerning fatigue corking on the left nacelle back up overwing fitting flange fastener hole.[86] The director of Sriwijaya Air, Jefferson Irwin Jauwena, insisted that the aircraft was airworthy. Although a 30-minute filibuster was noted, he insisted that the cause was bad weather, specifically heavy rain, rather than mechanical failure. In response, the NTSC said that they would exist coordinating with the Meteorology, Climatology, and Geophysical Agency (BMKG) in relation to weather in the Jakarta area.[87] However, the Ministry building of Transportation afterward examined the aircraft's airworthiness and determined that the shipping was rubber to fly.[88]

An Indonesian aviation skillful said that the shipping had been stored for repairs past Sriwijaya Air between 23 March and 23 October 2020, signifying good maintenance history.[27] However, other experts speculated that the long time spent inactive may have acquired deterioration and that technical bug may accept developed.[28] [89]

On fifteen January 2021, NTSC announced that the data from the FDR take been successfully downloaded. A total of 330 parameters were being examined and analysed by investigators.[90] A Reuters report said that the data extracted, such every bit flight path, speed and engine condition, were "in practiced condition".[91] [92]

The preliminary report was released on x February 2021.[25] [93] [94]

On 12 Apr 2021, the NTSC announced that all information from the CVR was successfully downloaded and included the blow flying.[95]

The investigation conducted simulations of the accident based on the FDR and CVR data in flight training simulator. The starting time simulation endeavor was conducted at the Las Vegas Flight University in Henderson, Nevada, on 27 October 2021. Information technology was found that the simulator did not react similarly to the accident flight, during an disproportionate thrust event. The simulation revealed that the thrust lever console was not closely monitored past the pilots. The investigation repeated the simulation session in NAM Training Centre in Jakarta on 7 December 2021 and successfully recreated the accident flight. However, some objectives could not be achieved due to the difference in configuration between the simulator and the accident aircraft.[95]

On 17 January 2022, the Indonesian National Transportation Safety Committee (NTSC) released the get-go interim argument report and indicated that they are currently planning to publish the terminal report no later than Jan 2023.[95]

Conditions [edit]

Superimposed ADS-B flying contour and weather information from BMKG

Weather data retrieved from BMKG confirmed the presence of moderate to heavy pelting during takeoff with risk of thunderstorms reported. The data later showed that a 15 km (9.3 mi) high cumulonimbus cloud was nowadays around Soekarno-Hatta International Airport with the minimum temperature of the deject tops at −70 °C (−94 °F), prompting speculation that the shipping had encountered turbulence. Visibility was reported to be 2 km (1.2 mi).[96] Analysis by the Indonesian National Found of Aeronautics and Space (LAPAN) showed that weather conditions were not extreme. LAPAN stated that a meso-convective system had been formed near the Coffee Sea at 11:00 WIB (04:00 UTC), but the system had already dissipated past the time Flight 182 took off.[97] According to NTSC, there was no indication that there would exist whatsoever significant cloud on the shipping'southward flying route. NTSC confirmed, however, that the pilots had contacted Jakarta ATC to change its heading to 075 due to bad weather.[25]

Maintenance issue [edit]

According to Tempo magazine, sources close to the investigation committee revealed that the shipping involved in the accident had a recurring autothrottle trouble for at least a month. NTSC, however, stated that they did not have the maintenance data nonetheless.[98] According to a person familiar with the investigation, "the autothrottle was producing more thrust in ane of the two jet engines" during the flying. If pilots did not set the ability manually, "an aircraft could turn to the side or even make an abrupt descent".[99] On 22 January it was revealed that a malfunction of the autothrottle organization had been reported several days before the flight; the aircraft can wing without the system, however, as the pilot tin can set the throttles manually.[100] [101]

The investigation will be focused on the faulty autothrottle organisation, equally multiple parameters from the FDR indicated that an aberration had occurred in the organisation.[102] NTSC later stated that the committee take sent 13 parts of the aircraft's component, including the aircraft's authothrottle system, to United states of america and United Kingdom for further examination.[103]

The maintenance logbook of the shipping revealed that there had been recurring bug on the Mach/Airspeed indicator and the auto-throttle system of the shipping. The Mach/Airspeed indicator was somewhen replaced on Jan iv, while the auto-throttle system was stock-still on January 5. The FDR data, however, confirmed that the automobile-throttle had malfunctioned again on January nine, the day of the accident. While the shipping was climbing through 10,000 ft, the left engine thrust lever continued to decrease, while the thrust lever on the right engine remained in identify. When the aircraft reached 10,000 ft, an aircraft upset happened. The autopilot disengaged and the aircraft rolled to the left with a bank angle of more than 45 caste. Few seconds later, the automobile-throttle then disengaged.[25]

Responses [edit]

In the firsthand backwash of the blow, the state's insurance company Jasa Raharja appear that information technology would compensate the relatives of the passengers and coiffure members aboard Flying 182.[104] Each next-of-kin of the deceased would receive Rp 50 million (US$3,530). Government minister of Social Affairs Tri Rismaharini appear that her ministry would requite Rp 15 million (US$one,060) for each victim for compensation.[105]

Delegations from Indonesia's Business firm of Representatives visited the operation center in Tanjung Priok. They afterwards announced that the House would hold talks with the Ministry of Transportation about the accident. They also stated that they would concord talks with BMKG, Sriwijaya Air and NTSC. The Indonesian Firm of Representatives will scrutinize the operation of comport of the Indonesian Ministry building of Transportation regarding supervision of airliners' compliance with the aircraft's maintenance manual. A total evaluation of every airliner in Republic of indonesia was afterward ordered.[106] [107]

The Regent of Thousand Islands, Junaedi, stated that the government of M Islands will build a monument dedicated to the victims of Flight 182 on Lancang Island.[108] On 22 January 2021, family unit and loved ones of the victims were invited to spread flowers approximately where the plane had crashed. Prior to the outcome, the managing director of Sriwijaya Air expressed condolences to the victims, and invitees were asked to pray based on their own behavior.[109]
